At Nurturing Souls Care, we aim to make the referral process clear, responsive and supportive for placing authorities, social workers and commissioning teams.
Our approach is collaborative, transparent and focused on achieving the right match for each young person.
The process

Initial Enquiry
Referrals can be made by local authorities, social workers or commissioning teams by contacting us directly. At this stage we will request key information about the young person, including:
• Young person’s profile and current placement circumstances
• Presenting needs, risks and support requirements
• Legal status and care history
• Education arrangements
• Safeguarding considerations and any immediate concerns
• Desired outcomes for the placement
This allows us to consider whether we can meet the young person’s needs safely and effectively.

Referral Review & Matching
Our management and care team review the referral information in detail to ensure the proposed placement is appropriate for both the young person and the existing residents. We carefully assess:
• Compatibility with the home environment
• Staffing capacity and skill requirements
• Risk management considerations
• Therapeutic and emotional support needs
• Education and development planning
Where helpful, we will arrange a professional discussion or Teams meeting to explore the referral further.

Placement Decision
Once the assessment is complete, we provide a clear decision on whether we can offer the placement, along with proposed staffing arrangements, support strategies and expected outcomes. If accepted, we agree:
• Placement objectives
• Care and risk management plans
• Reporting arrangements and review schedule
• Admission timeline

Admission & Transition
We work closely with the placing authority and the young person to ensure a smooth, supported transition into the home. This includes:
• Welcome planning and preparation
• Initial care planning meetings
• Introduction to key staff and routines
• Ongoing communication with professionals and family
Ongoing Review & Partnership
Throughout the placement, we maintain open communication and structured reviews with the placing authority to ensure the placement remains stable, purposeful, and effective. We provide:
- Regular progress updates and reports
- Statutory reviews and planning meetings
- Continuous evaluation of outcomes and support needs
